WGH Receives Patient-Centered Medical Home Accreditation by TCT

Wabash General Hospital’s Rural Health Clinics were recently accredited for Patient-Centered Medical Home (PCMH) by The Compliance Team (TCT), a Medicare-approved accreditation organization.

After successful completion of the program, Wabash General received Exemplary Provider® status, TCT’s exclusive accreditation designation.

“PCMH requires a team that is truly focused on its patients and their outcomes,” explained Kristen Ogden, Director of Quality Improvement for The Compliance Team. “Wabash works hard to put patients first and improve processes to better serve their communities. Wabash is highly deserving of the distinction of Exemplary Provider!”

The PCMH model puts special emphasis on health maintenance, preventative screening, multi-specialty medical services, patient experience reporting, and benchmarking. Patient-centered programs and value-based care are advanced by the National Academy of Medicine and the nation’s leading primary care physician associations. PCMH accreditation is official after an on-site survey to verify the clinic is meeting specific quality standards and is valid for a 3-year period.

“I am extremely proud of each of our rural health clinics,” stated Andrew Kleinschmidt, VP of Professional Services with Wabash General. “Our providers and staff give intentional focus on wellness and prevention and on pushing through barriers of care on behalf of their patients while still meeting their acute needs. The extra steps to become accredited as a Patient-Centered Medical Home validate those efforts and will continue to hold us accountable to the highest level of service to our patients.”

Wabash General Hospital District is a vital part of the communities they serve and has been serving the healthcare needs of Illinois since 1951. With over 500 employees providing continuous quality improvement and exceptional patient-centered care, Wabash General is well positioned to maintain expansion of services to those who need it most.
